British Airways does have a VERY long list... I think they were aiming at "all the titles that might be present on your passport, because we are going to insist that the title+name on your ticket and the title+name on your passport are the same" or something along those lines.

At the short end I once booked a hotel in Amsterdam that had only Mr and Mrs (in lots of European countries all adult women are the local version of Mrs and I guess the translator had just translated the words)

To the option of saying "well why do I even care, I'll just use your name and not your title" the problem occurs that some people (m'mother) really really HATE being addressed as "name" rather than "title name". It might be better in such cases use neither (begin "dear customer", with no personalisation, for instance)

To the problem of "people typing offensive shit into the title box" - surely this problem exists just as much with the name box - perhaps it would be better to attempt an offensive shit detector which could then be used on both? Or a "is that really you" detector.
